Overview

The program combines personal training, group workouts (group dynamics), healthy eating and online coaching. All participants — company, participant and coach team — sign an agreement committing to a weight-loss goal of up to 3 BMI points. If the goal is reached, Zilveren Kruis pays the bill; if not, the participant or coach team pays according to who didn't hold up their part of the deal. This responsibility mechanic drives higher commitment and better results.

Officially launched in 2014, the program grew to more companies including Shell, KPN, KLM and MSD.

Challenge

Develop a healthy-lifestyle program that leads to a sustainable business model: reduced healthcare cost, reduced sick-leave cost, higher efficiency, a motivational financial reward model, and a measurable, viable business model. A pilot was developed in 2012 with Zilveren Kruis, Achmea Health Centers and Radboud University, rolled out at four collectively insured companies: Arriva, KPN, Rabobank and Sita.

Pilot results

202participants with obesity/overweight
58%dropped from obese to overweight (~€399/yr sick-leave saving)
9 cmaverage reduction in abdominal circumference
33–75%drop in diabetes risk (~€7,000/yr cost)
17–20%drop in heart failure risk
5–15%drop in colon cancer risk

Estimated saving: ~€2,500 per participant per year.

The science

Together with the Achmea Knowledge Center and Radboud University Nijmegen, exploratory research examined potential cost impact, health categories and effect on disease risk and sick leave. Evidence supported a 3-point weight reduction for obese participants and its health impact — enough to continue developing the pilot into the current business and coaching model.

The network

Certified fitness centres with Personal Trainers (level B) and registered dieticians — starting with the network of Achmea Health Centers, now a mix of chains and single-location fitness and health centres.

Current program

Developed by Achmea Health Centers, now coordinated at Achmea Vitaliteit B.V. — the Achmea entity for vitality programs — and offered by Zilveren Kruis Achmea to specific collectively insured companies.
